Aloft Hotel, 3939 Duckling Drive – During a routine inspection on July 30, inspectors observed milk cartons in the back of the front cooler case at 53 degrees; and fryer baskets with bent wire that was breaking off.
Avalon Recreation Association, 510 Heathfield Road – During a routine inspection on July 30, inspectors observed black buildup in the ice chute of a soda machine; and no chlorine sanitizer test strips.
Bon Secours St. Mary’s Hospital, 5801 Bremo Road – During a routine inspection on July 31, inspectors observed multiple dented cans, including four cans of fruit with dents along the bottom seams and a can of peaches with a deep dent; two deli slicers stored with dried food debris along their edges, in crevices and on the blades; cut lettuce in a serving-line make table at temperatures ranging from 40 to 52 degrees, with lettuce filled at least two inches above the metal bin’s fill line, and cut watermelon in a display case at 43 degrees in the front row and 38 degrees in the third row; various TCS foods cooling in tightly covered bins; gnats on walls around the dish machine and above a hand sink; what appeared to be black mildew between dispenser nozzles at the self-service coffee creamer station, grime buildup on refrigeration unit gaskets, a dusty fan guard in a drink walk-in cooler, grease accumulation inside oven doors, white fuzzy grime on a shelf in the meat walk-in cooler and food debris on produce walk-in cooler racks; leaks from the mop-sink and two-compartment sink faucets and a slow-draining hand sink in the grill serving area; direct plumbing connections at prep and three-compartment sinks throughout the kitchen; chipped floor tiles, areas of low grout, broken baseboard tiles throughout kitchen and peeling wall paint in the dish area; and what appeared to be dark-colored mildew on ceiling tiles above the dish-machine entrance, with steam billowing from the machine.
Casa Italiana / Pizza Square: Italia Market, 8801 Three Chopt Road – During a routine inspection on July 30, inspectors observed cooked rice in the walk-in cooler dated July 22; scratched and stained cutting boards at Pizza Square; a slow leak from the bar hand-sink faucet; and several chipped floor tiles at the bar.
Clean Eatz Richmond, 11801 West Broad Street #1B – During a follow-up inspection on July 30, inspectors observed that soap was still unavailable at three of the facility’s four hand sinks; sanitizer at the three-compartment sink again measured 0 ppm; diced tomatoes in the prep unit measured 45 degrees; gasket liners remained torn on both doors of the prep unit; vents in the walk-in cooler remained dusty; and a Y-valve remained attached to the mop-sink faucet without an appropriate backflow-prevention device.
Courtyard by Marriott, 10077 Brook Road – During a routine inspection on July 31, inspectors found that the person in charge was unable to provide an employee health policy; food debris was observed on dishes on the bottom rack of the clean-dish rack; dried food debris was observed inside two microwaves; dried food debris was observed on a can-opener blade; boiled eggs measured 57 degrees and liquid egg 54 degrees in a mini refrigerator with an ambient temperature of 55 degrees, and cheese stored on ice measured 50 degrees with only the bottom of the pan contacting the ice; the mini refrigerator had an ambient temperature of 55 degrees, the bar prep cooler had an ambient temperature of 46 degrees, chemical-storage and walk-in-cooler racks were rusted, and a rusted bolt and rust were observed inside the kitchen ice machine; three white and one brown cutting board had deep scratches or staining; no irreversible registering temperature indicator was available for the high-heat dish machine; dried food debris was observed on a walk-in-cooler gasket and dust on walk-in-cooler fan guards; a hand sink had its hot water turned off because of a leak; and dried food debris and dust were observed on floors beneath storage racks and cookline equipment and tables, while dust and dried food spills were observed on ceiling tiles and vents.
Denny’s, 6598 West Broad Street – During a follow-up inspection on July 31, inspectors observed a large number of fruit/drain flies in the kitchen, with sources including dirty floor drains and hand sinks; stained and scored cutting boards; deep fryer baskets in disrepair and an inoperative light in the hood system; a direct plumbing connection beneath the three-compartment sink; a missing floor tile in the staff break room, a broken floor tile near the dish machine and damaged floor coving in the rear dining room; and greasy hoods.
Domino’s Pizza, 5526 Lakeside Avenue – During a follow-up inspection on July 30, inspectors observed a slow-draining hand sink with food debris inside; dried food debris buildup on the oven, exterior of the make table and computer/safe at the front of the establishment; a leak at the three-compartment sink; and food-debris splatter on ceiling tiles and dusty ceiling vents.
Holy Grounds, 5801 Bremo Road – During a routine inspection on July 31, inspectors observed black mildew on the sides of the ice machine; whipped cream on the counter at 64 degrees; rusted racks in refrigeration units and a dish machine flashing an E7 error code, with its display showing a 165-degree wash cycle and 177-degree rinse cycle while an irreversible temperature-measuring device reached 170 degrees; and a direct plumbing connection at the three-compartment sink.
Jing Sushi, 7019-G Three Chopt Road – During a routine inspection on July 31, inspectors observed a scratched and stained cutting board at the sushi bar.
Microtel Inn & Suites, 6000 Audubon Drive – During a follow-up inspection on July 30, inspectors found that breakfast items were not being sanitized; and no sanitizer or sanitizer test strips were on site.
Red Salt, 12221 West Broad Street – During a routine inspection on July 31, inspectors found that multiple employees had not signed the employee health policy and their training on illness-reporting responsibilities could not be verified; shellstock tags were missing the dates products were last sold; parasite destruction documentation was unavailable for yellowtail, tilapia, squid and sea urchin; raw chicken was stored next to raw beef and raw onions in a make table; chlorine sanitizer at the dish machine measured less than 10 ppm, with air bubbles in the dispensing line and the line not fully submerged in the solution; cream cheese measured 45 degrees and spicy mayonnaise 46 degrees in a sushi unit, sliced cheese measured 47 degrees, asparagus 46 degrees, mushrooms 44 degrees, raw burger 45 degrees and raw steak 46 degrees in one cookline make table, and orzo measured 45 degrees, pasta 47 degrees, spinach dip 45 degrees and potatoes 46 degrees in another make table; garlic butter dated July 23 had not been discarded within seven days; crab dip cooling in an ice bath on the counter measured 80 degrees after one hour and 40 minutes; a few small flies were observed at the bar; a sushi unit and two make tables were not maintaining proper temperatures, a fryer basket was damaged and appeared to be missing metal from a corner, walk-in storage racks were rusty and a CVAP drawer gasket was damaged or missing; grease buildup was observed under fryers; and a hole was observed in the wall near the rear door, floor and coving tiles were damaged near the rear door, beneath the fryers and behind the dish machine, and grout was low in those areas as well as at the bar in front of the dish machine and near the walk-in cooler door.
Rico’s Mexican Grill, 11321 Nuckols Road, Unit D – During a follow-up inspection on July 31, inspectors observed carnitas dated July 25 and cooked meat dated July 24 that had not been discarded as required following walk-in refrigerator temperature problems, despite instructions to discard the foods during a July 29 inspection.
Rise Biscuits & Donuts, 11561 West Broad Street – During a routine inspection on July 30, inspectors observed biscuit gravy in the steam table at 134 degrees; sausage patties at 43 degrees and raw chicken at 47 degrees in a drawer refrigeration unit, and unsalted butter on the counter at 78 degrees; an unlabeled container of brown sugar; torn gasket liners on both doors of the kitchen prep unit, door of the cookline lowboy freezer and both doors of the lowboy refrigerator beneath the biscuit-making station, and a fryer basket with broken-off metal pieces; a scored cutting board; old food debris on wire racks in the walk-in cooler and dry-storage area; a Y-valve on the mop-sink faucet without an appropriate backflow-prevention device; loose trash and debris around the dumpster area; and two mops stored head-down and unable to air dry properly.
Shagbark Restaurant, 4901 Libbie Mill East Boulevard – During a routine inspection on July 30, inspectors observed a staff member slicing bread at the beverage station and eating a slice during preparation; multiple containers of moldy strawberries in the walk-in cooler; fryer baskets in disrepair; heavily scored and stained cutting boards; dirty, moldy or black-stained refrigeration gaskets throughout the kitchen, dirty or moldy walk-in cooler fan guards, and a dirty bar-area beer cooler door and gaskets; dried food residue on kitchen walls, dirty walls beneath the three-compartment sink, dusty ceiling tiles and dusty ceiling vents; and dirty/greasy hood filters.
Subway at Fas Mart, 2650 New Market Road – During a follow-up inspection on July 30, inspectors observed chicken at 57 degrees, tuna salad at 54 degrees, diced chicken at 55 degrees, tomatoes at 55 degrees and spinach at 55 degrees, with air temperatures measuring 60 and 55 degrees.
Thai Won On, 3422 Lauderdale Drive – During a routine inspection on July 31, inspectors observed green curry sauce in the walk-in cooler at 45 degrees; a stack of takeout containers stored on a prep shelf with their food-contact sides facing upward; and wire racks in the walk-in cooler covered with old food debris.
The Daily Kitchen & Bar, 12201 West Broad Street – During a follow-up inspection on July 31, inspectors observed a server reach barehanded into a bowl of fries in the pass-through window and eat the fries; cooked chicken at 47 degrees, tomatoes at 52 degrees and pork at 50 degrees in a sauté unit, raw shrimp and heavy cream at 43 degrees, and lettuce at 49 degrees, turkey at 48 degrees, tomatoes at 48 degrees and quinoa tabbouleh at 43 degrees in a salad unit; and sauté and salad units that were not maintaining TCS foods at 41 degrees or below, with missing or torn gaskets on both units.
Yoshi Sushi, 1601 Willow Lawn Drive – During a follow-up inspection on July 30, inspectors observed shell eggs stored above produce in the walk-in cooler.
Zaiqa Indian and Pakistani Cuisines, 3409 Old Parham Road – During a routine inspection on July 30, inspectors found that the person in charge was unable to produce a verifiable employee health policy; the person in charge was unable to produce a vomit-and-diarrhea cleanup policy; a hand sink behind the counter had its hot water disconnected; various TCS foods in the walk-in cooler and lowboy units lacked date-marking labels; multiple spray bottles containing cleaner were unlabeled; the rear door was open; numerous flies were observed throughout the kitchen and dining room, and roaches were observed in kitchen and storage areas; single-service cups were being used as scoops in dry ingredients; single-service utensils at a self-service area were stored with handles in multiple directions, leaving lip-contact surfaces unprotected from contamination; fryer baskets had bent corner wires and a spice rack was rusted; cutting boards at make tables were scratched and heavily stained; dried food debris splatter was observed inside microwaves, food debris and grime on refrigeration unit gaskets, and a rack between lowboy units was dirty; a Y-valve at the mop sink had two connected hoses with the system under pressure; a direct plumbing connection was observed at the three-compartment sink; and a storage room was filled with unused equipment and boxes, while an unused dish machine also was observed in the kitchen.
