KITCHEN MECHANICAL DISHWASHER: The Kitchen Manager and CFM Jessica Chadwick informed me that the dish machine was switched to a chlorine sanitizing machine since the food-contact surfaces of equipment and utensils did not reach 160 degrees F during the sanitizing cycle of the high temperature dishwasher-her thermometer registered 154F. However-today no chlorine chemical sanitizer residual was detected in the final sanitizer rinse it must be between 50-100 ppm as required. The Wash Gauge registered 160F and the Rinse Gauge Registered 184F. These temperatures are too high for chlorine sanitizer to be detected. The Rinse Gauge is not accurate since the dish surface is only 154F and not at least 160F. THIS MACHINE MUST BE SERVICED IMMEDIATELY TO EITHER WORK PROPERLY AS A LOW TEMPERATURE CHLORINE MACHINE OR A HIGH TEMPERATURE SANITIZNG MACHINE. CORRECT IMMEDIATELY. You cannot use this machine until repaired. You must immediately start manually washing, rinsing, and sanitizing all dish wares and/or use disposable dish wares. No chlorine sanitizer test strips were on site to determine appropriate sanitizer concentration of the dish washer. Supply ASAP.

Raw animal foods were relocated. Train staff. Raw chicken was stored above raw beef and seafood. Chicken was relocated. Train staff of correct storage order for raw foods. Foods stored in the chest freezer and some foods in the cook line coolers were stored open with no covering. Cover all foods. Raw chicken touching raw beef in common equipment with no barriers to prevent cross-contamination. Foods were discarded. Train staff. Observed wet wiping cloths throughout kitchen not being stored in sanitizer solution. Obtain sanitizer buckets. Keep wet cloths inside buckets. Two food employees were observed washing their hands at the 3-compartment sink rather than the designated handwash sink. Additionally, no soap was used. Train staff. Food Employee observed changing tasks that may have contaminated hands without a proper handwash in-between. Employee observed taking out trash, returning inside kitchen and not changing gloves or washing hands. EHS instructed employee to wash hands. Train staff. Food ingredient storage containers (dry goods), in the kitchen, are not all labeled with the common name of the food. Label all food ingredient storage containers. A fan with a mist observed at the cook line. Mist water can contaminate foods. Remove fan. Employee personal items such as vapes, keys, and cell phones were observed in food preparation/storage areas. Employee personal food also observed stored intermingled with retail foods. Create a designated area for employee personal items. Label area. Observed large pot stored outside subject to contamination. Two oversized pots observed at facility must be removed. The dishwasher and 3-compartment sink are not large enough for ware washing these items. Knives used to cut raw chicken were rinsed off with water and returned to knife strip without washing with soap and using sanitizer. EHS addressed employee. Knives were removed from strip to be washed, rinsed, and sanitized. Knife strip was also sanitized during inspection. Train staff on correct ware washing procedure. Wash with soap, rinse with water, sanitizer (bleach and water at proper concentration), and air dry. Food observed stored under fan system in walk-in cooler that was significantly dripping water. Foods stored under the system and observed with water were discarded. Shelf under fan system cannot be used to store foods until repaired. Service unit immediately to stop dripping. Send proof of service and correction to the Department. Food dispensing utensils in the dry goods stored in the food and not with handle above the top of the food. Remove bowls used as scoops, replace with scoops, and always store with the handle above the food. Observed in-use knife stored inside cut produce with the handle touching the foods. Knife was removed during inspection. Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours, located in all of the coolers, are not being date marked. Foods must be marked with the date of preparation or the date to discard (7 days after preparation). Train staff. Yum yum sauce, mayo, and spicy mayo were held at 78°F, in the cook line area, rather than 41°F or below as required. These foods were stored outside of refrigeration. Foods were discarded. Train staff. Mayo and raw meats stored on skewers were held at 45-53°F, in the prep top at the cook line, rather than 41°F or below as required. These foods were stored outside of well or sticking above wells (food on skewers). Foods were discarded. Train staff. Several gallons of milk, eggs held at 54°F, in the under counter cooler in the front drink area, rather than 45°F or below as required. Milk was discarded. Unit cannot be used to store TCS foods until it can maintain foods at 41F or below. Repair/service/adjust unit. Observed cutting board at cook line with deep scoring. Replace. Also observed wooden handled utensils in disrepair above 3 compartment sink. Replace. Observed food containers stored directly on the floor in walk-in cooler, rather than 6 inches off of the floor as required. Move all foods 6 inches off of the floor as required. Facility is washing and reusing single use food containers (ex: mayo for spicy mayo, seaweed containers, soy sauce buckets) for food storage and storing utensils/condiments. Replace with approved reusable food grade containers. Food preparation sink does not have an adequate air gap of at least 1 inch, between the water supply and flood rim of the sink. Install air gap. Employee observed washing dishes in the rinse compartment with raw chicken debris observed inside sink. EHS trained staff on proper ware washing procedure. Wash with soap, rinse with water, sanitizer (bleach and water), air dry. Compartments were cleaned and sanitized during inspection. Ensure staff compliance. Food facility inspection indicates evidence of insect activity, but facility does not have a pest control program. Flies observed in kitchen and fly traps/control devices were observed in kitchen. Remove all residential pest control devices. Facility must be serviced by a pest control professional. Back door to the outside located in the kitchen is being propped open. Screen door is in disrepair and has large gaps allowing insect entry. Repair screen door and keep both doors closed. In Nazareth & IHM: Apples observed in service areas without “wash before eating” signage or without being washed and individually wrapped, risking contamination. Mango lassi and condiments stored in the front cooler by the order counter (Time/Temperature Controlled for Safety (TCS) ready-to-eat food prepared in the food facility and held for more than 24 hours) is not being date marked. Broken/unused equipment stored in front area must be removed from food facility. Test strips used to determine appropriate sanitizer concentration are expired. Replace chlorine test strips. A Chester County Health Department Certified Food Manager is displayed, but the individual is no longer employed by the food establishment. Apply for a CCHD CFM using Gharti’s CFM. The light intensity in the food preparation area is not at least 50-foot candles. Increase lightening intensity in the kitchen by adding approved light fixtures or replacing bulbs. Avalon Restaurant, 116 E. Gay St., West Chester, Sept. 9. Fail. Dishwasher in Whiskey Bar was not dispensing proper concentration of chlorine. During the inspection, chlorine container was changed with a new container; machine was primed and then properly dispensing the proper concentration at approximately 50 ppm. Squeeze bottles and a spray bottle were not properly labeled. This was corrected during the inspection. Make sure all containers are properly labeled. Atosa refrigerator was observed at 48°F rather than 41°F or below. Internal food temperatures were observed at 41°F. Food was immediately removed and placed in other refrigeration. Frozen Tortellini in small countertop Atosa freezer was observed defrosted and ice build-up in freezer. Tortellini were discarded during the inspection. Frozen food must be kept frozen solid. Do not use until repaired. Repair immediately. Walk-in refrigerator was observed with an ambient air temperature of 48°F; however, internal food temperatures were observed at 44°F. Refrigeration repair was called during the inspection and walk-in refrigerator was repaired blowing air at approximately 41°F. Refrigeration technician said he would be back tomorrow morning to check walk-in refrigerator and repair other refrigeration. Air vents in kitchen were observed dusty. Clean air vents within 24 hours. Floor below deep fryer was observed dirty from grease. Clean within 48 hours. Plumbing below three compartment sink in the kitchen was observed to be leaking. Repair within 14 days.

The handwash sinks were blocked and not accessible at all times for employee use in the following locations. 1- Hand sink at bubble tea area, handbag in sink. 2- Cook line hand sink in mop closet covered with aprons and brushes. Maintain this area clear as discussed. Staff corrected on site. The following equipment not observed in good repair; 1- Fire / Ansul tags dated July 2025, maintain current dated within once calendar year. 2- Wok Bain maire & adjacent undercounter fridge observed at 58-62 F. Discontinue use, and have serviced ASAP- Repair or replace. 3- Replace two dishwasher racks as discussed. 4- Defrost chest freezer in middle hall way behind prep room. Several Foods in the walk in freezer stored open with no covering. Cover product as discussed. Observed wet wiping cloths in kitchen, not being stored in sanitizer solution. Sani bucket set up properly at server station. Pick up loose trash bags behind dumpster. 1- Build up on dishwasher racks, a food contact surface, was observed to have food residue and was not clean to sight and touch. Replace with new dish racks. 2-Bain marie cutting board is screwed down to the table. These boards need to be washed and sanitized routinely throughout the day. Remove screws so the cutting board can be removed and cleaning can be completed on the top and under side of cutting board. Observed food stored directly on the floor in the walk in freezer, rather than 6 inches off of the floor as required. Clean and purge entire walk in freezer including floor and shelving. 1- Cook line bain maire at Wok station was held at 58-62°F rather than 41°F or below as required. All products including shrimp and raw protein discarded into trash can. Have unit serviced ASAP – Discontinue use until unit is repaired. 2-Open can of Honey BBQ in middle prep room left open and not placed into refrigeration per manufacturer’s requirement labeling. Discarded. Retrain staff. 3-Garlic shallot sauce observed at room temperature at server station. Once garlic is chopped and minced, it is a temperature time control food that needs to be refrigerated or discarded every 4 hours. Retrain staff, enforce through Managerial team. Bain maire on cook line Fryer side, observed with product stored above the upper bins which is not the proper cold hold storage practices. Eggs observed at 54 F – All product decarded that was observed stored in correctly. Inform staff not to store food in cold hold units this way, enforce daily through managerial team. Non-food contact surfaces not cleaned at a frequency to preclude accumulation of buildup, crumb, and splash; 1- Walk in freezer floor – Purge and organize. 2- Fan Guards in the walk-in cooler. 3- Cook line floor under wok station. 4- Ice chutes (2) located in front bubble tea and server station 5- Soiled trays with food debris left on dolly in middle prep room. These trays need to be cleaned after use, enforce through managerial team. Facility cooks chicken on site, then freezes it immediately- chicken is dated for when it is cooked and frozen. Facility is to also provide the date the chicken is placed into refrigeration to ensure it is used within 7 days. Person in Charge provided containers of chicken with dates of when they were placed in the cooler. The food facility does not employ a certified employee as required.
