For the life of me, can’t figure out how I keep killing tomato plants
Is it under watered, a disease, didn't see any pests

I’m confused…. It looks like there are multiple tomatoes in that container, and the container is too small for 1 tomato. By this time it should be in the ground or a much larger container, and alone. Tomatoes are greedy plants, but their greed is what makes them produce so much wealth.
It could be more that that, but that was my first thought. Good luck!

What kind of dirt is this? Garden soil and dirt out of the ground are far too heavy for a container. The soil compacts.

That doesn’t look like the correct soil for that container.

You need to tell us a lot more about your plant’s environment, otherwise we have to take pot shots and give you possibly bad answers.
The fake bokeh is hurting visibility.
Are you in the tropics?
The pot’s soil seems to have zero aeration and drainage (perlite and other functional rocks).
The pot’s soil seems bone dry. And packed. But we can only see the top layer…
How large is your pot?
Give us every single detail you can think of! Maybe we can help. But you have to help us help you.

The tomato is a rustic plant but at the same time, due to the many hybridizations practiced by trade, it needs a lot of attention If the plant is not treated preventively against fungal diseases, it will surely get sick. If it is not grown in a suitable substrate it will not develop well. If it is not watered properly it can suffer and even die. If not pruned regularly it will produce small and suffering fruits and will tend to get sick more easily.
