Never put raw mushrooms on pizza, but that’s just a misdemeanor. As are bad slices. I’ve seen way worse.

they made it too big or misshapen or most likely there was a hole so they cut a piece, perhaps even a straight piece the full diameter, out of the middle then proceeded to tidy it up and you got left with a trimmed down slice

There was probably a hole in the pie, caused by thin dough sticking to the floor of the oven. When they scrape under the dough to turn the pie, the metal will catch on this thin spot and tear through the pie. Instead of completely re-firing the pizza, some cooks will innocuously cut out a section to remove the hole. One of the cooks at the pizza place I worked in would do this, but instead he’d remove a strip along the diameter so it wasn’t obvious that anything was missing. We’d always throw our pies to be a little bigger anyway, so the missing piece never actually detracted from the “14 inches” as labeled.
My guess is this is a really terrible attempt at removing a hole.
