Had this happen a couple times. I think WE are aggressive lid flippers. However, I am on my second lid, and third or maybe fourth time putting it back on.
I also need to rebuild the cart.
I have been told no Father’s Day steaks without the egg fixed, by a very aggressive flipper.

no matter how aggressive your lid flipping is, there should be no reason that your dome comes off. Ive hucked open my egg drunk as a skunk before and never even considered it would come off. Make sure your dome/bandset is installed snug per BGE instructions, I’ve had my dome on its band for 7 years now since I upgraded from old bandset to new and never has this happened. It looks like the lower band base attachment might be the problem since it the dome still has it on in your pictures
You could run some ultra high temp RTV around the bandset during reinstall and that should combat your “dome flinging/launching.” Or at least its a bandaid on a larger issue of how you treat the egg.
While it is a treasure chest of meat sometimes, maybe try not to open it like one.
Are you tightening the retention bolts on the back of the band to the point that they bend? If not, then you need to do that, it’s possible they’re not tight enough and the band slips off. Without any more context, that’s the only thing I can think of.
